Built by the team behind Evry Health's member platform · HIPAA-native from the first commit

Risk moves faster than a spreadsheet. Gap reports age for weeks while nurses grind call lists, chase records, and re-document the same visit in three systems.

Bellwether keeps every member moving. Agents stratify risk nightly, draft the outreach, and clear the routine — your clinicians see evidence and approve at a glance.

A chart a nurse can actually read.

Every code translated, every event on one humanized timeline — claims, labs, Rx, ADT — with the AI summary cited line by line.

1,204 members → cohort built · 3 rules

Ask for a cohort in plain English.

Natural language becomes an auditable rule set you can inspect, edit, and save — then hand to an agent to work.

Get ahead of every readmission.

Agents watch ADT feeds and predictive risk, then open the case, do the prep work, and escalate with context — before the 30-day window closes.

  1. 01
    Connect

Claims SQL, FHIR R4, HL7 ADT, lab SFTP drops. No rip-and-replace; every ingestion step logged for audit.

  2. 02
    Stratify

Nightly measure runs and risk models sort your whole book — rising risk surfaces before it becomes an admission.

  3. 03
    Act

Agents work the routine: outreach drafts, scheduling nudges, record requests, adherence follow-ups.

  4. 04
    Approve

Anything touching a member waits in the review queue as an evidence card. One glance, one click, fully logged.

Bellwether doesn't just run agents. It hosts yours.

The agent workforce inside Bellwether is only half the story. The other half: any agent your teams or vendors run can become a governed user of your population data — discovered, approved, scoped, and audited like a member of staff.

Agents register like employees

Any agent — your data-science team's notebook, a vendor's bot, something you built last weekend — discovers Bellwether the way agents should: a public auth.md generated from the live scope catalog. Registration lands with your org admin. Approval assigns an agent group and credentials scoped to exactly what the job needs — expiring, self-rotating, revocable.

No shared API keys. No side doors. Effective authority is always group permissions ∩ credential scopes.

De-identified by default. PHI under BAA. Both useful.

Agents without a BAA work a Safe-Harbor-scrubbed view of your book: pseudonymous members, no names, no free text, no way to ask for them. They still find the signal — rising risk, missed care, data anomalies — and publish analyses in pseudonyms.

Your staff, or a BAA-attested PHI-tier agent, re-identifies the members behind an analysis with one explicit click — logged as PHI access, gated on phi.view. The insight travels; the PHI doesn't.

Take your book to any agentic platform

Point whatever runtime you like at Bellwether — a Claude agent, an analytics copilot, the platform your vendor ships next year. They get governed access to the same cohorts, measures, and analyses your staff see, at the tier you set.

And nothing external touches a member directly: any outreach an outside agent proposes lands in the same maker-checker queue as our own agents, waiting on a human with the right to say yes.

The new way to run population health vs the suite you tolerate and the headcount you can't hire.

Built for PHI. Governed like it matters.

Immutable audit

Every PHI read, every AI decision, every human approval — one append-only ledger with model version and evidence links.

Granular RBAC

Role and module permissions enforced in the UI and on the server, down to field-level PHI masking per role.

Agent governance

External agents get scoped credentials and a HIPAA tier. De-identified by default; PHI only with an audited BAA. Nothing reaches a member without human sign-off.
